Reaching Resources connects seniors with aging resources in Hamilton County.
This program offers a resource guide, advocate, and support for older adults in Hamilton County. Reaching Resources is the hub for all things relating to aging in our community. The goals of the program are to prevent isolation, improve quality of life, and help maintain independence.
How Reaching Resources Works
A Reaching Resources Coordinator will meet one on one with an older adult in their home to complete a comprehensive intake assessment on both needs and strengths today, as well as planning for the next three to five years. Caregivers are encouraged to attend the intake meeting. The Reaching Resources Coordinator will ask questions regarding physical health, current supports, financial status, specific requests, and other potential needs.
Then, the Reaching Resources Coordinator will assist the senior with action plan goals. The Reaching Resources Coordinator has up to date and accurate information about the resources, programs, and services for seniors in Hamilton County. They will help seniors with the improved referral process and connect them directly with the resource. Furthermore, they will be able to proactively identify needs during the intake interview to help determine what other services and resources the senior may benefit from in their area.
To receive assistance through Reaching Resources, you must be 55+ and live in Hamilton County.
